QuailRidge Pup Reservation Form

Enclosed is $200.00 U.S. as a deposit for a FEMALE MALE (Must Check One) puppy from the litter out of CH QuailRidge's Mesquite Mudslinger x CH QuailRidge's Harley Thyme. Balance due on the pup will be paid upon picking up the pup, or prior to shipping. I understand the deposit is non-refundable unless there is not a puppy available. If a puppy is not available the deposit will be refunded immediately. I will be informed within 48 hours of the whelping of the pups to inform me of the number of males and females born and pups availability. California State Sales tax applies to residents of California (7.25%).
Shipping charges for the pup are an additional cost to the buyer, which include the cost of a Health Certificate, shipping crate, and shipping charges due the airlines. Shipping charges are due prior to shipping. The Pup will have; dew claws removed, its first set of shots, be wormed, and will be examined by a veterinarian prior to shipping.
